We're looking for a compassionate and proactive Independent Living Co-ordinator to deliver high-quality housing management and support services within our Extra Care scheme. In this role you'll work closely with residents, colleagues and external partners to help individuals maintain their independence, sustain their tenancies, and feel part of a vibrant community. The role involves managing lettings, supporting residents' wellbeing, responding to day-to-day issues, and ensuring a safe, inclusive and well-run living environment.
A full UK Driving licence and access to a vehicle is essential for this role.
Specifically, in the role you will:
- Be responsible for letting empty homes at the scheme by participating in meetings of the multi-agency Extra Care Allocations Panel to consider and prioritise referrals, maintain a waiting list and allocate homes.
- Actively contribute to the allocations process to ensure a balance of need to create a vibrant scheme.
- Show prospective residents around the scheme and complete pre-Occupation Contract procedures.
- Manage empty homes at the scheme including end of Occupation Contract arrangements and liaising with Hedyn's Property & Place team regarding inspections and works being undertaken.
- Actively publicise the scheme to sustain demand.
- Undertake the sign-up procedure.
- Provide high quality housing management services by being aware of and implementing Hedyn's policies and procedures in relation to housing management and safeguarding, making referrals and attending meetings as appropriate.
- Liaise with residents and the Community Safety Team to address neighbour disputes and/or anti-social behaviour.
- Monitor and address issues relating to the general conduct of residents and their visitors including property condition.
- Keep property and residents' details updated on Hedyn's housing management system.
- Make residents aware of the end of Occupation Contract procedures.
- Provide unobtrusive support and assistance to residents that fosters a sense of independence by making contact to an agreed level with each individual resident.
- Ensure residents fully understand their rights and responsibilities as Hedyn residents and consumers of care and support services.
- Work in close partnership on a daily basis with the on-site care provider and Social Care to ensure that residents' needs are met and that joint, effective working practices are established and maintained.
- Monitor/visit as appropriate and review the needs of individual residents in collaboration with other services including the on-site care provider.
- Instruct new residents in the proper use of all equipment provided including the communal laundry, emergency alarm call system and Telecare installations.
- Provide a link between residents and voluntary and/or statutory support services which will include attending case conferences, multi-agency meetings and liaison with hospitals regarding the discharge process as appropriate.
- Ensure residents are aware of the relevant complaints and reporting procedures.
- Liaise with the Income & Inclusion Team regarding rent account queries from residents.
- Collect, reconcile and bank guest room receipts and other resident payments.
- Maintain an up-to-date TV licence for the scheme including completing the application process and collecting payments.
- Monitor energy consumption in relation to fuel bills.
- Supervise all contractors while they are on-site who provide communal services such as window cleaners, grounds maintenance and cleaners.
- Control the provision of heating in communal areas.
- Identify and report communal area repairs.
- Report repairs and monitor the performance of contractors when requested to do so by individual residents.
- Positively promote the health and safety of residents and visitors to the scheme by completing and maintaining records for residents with details of doctors, care provision, Telecare requirements, next of kin, key holders, medication and support workers.
- Refer into the assessment process for individual Telecare requirements.
- Supervise the security and safety of the building and on-site services.
- Test emergency systems as required by Hedyn's policies and procedures.
- Provide help in emergencies by responding to emergency calls and contacting the emergency services, care providers, doctors and next of kin as appropriate.
- Provide emergency first aid.
- Set up and maintain emergency procedures for cover arrangements including out of hours.
- Promote the scheme as a highly desirable place to live.
- Promote the use of communal facilities at the scheme by working in partnership with Hedyn's Communities and Regeneration Team and the on-site care provider and encouraging social activities on and off site.
- Support the establishment of residents' groups and social groups and provide on-going assistance as necessary.
- Encourage and support productive links with the local community.
- Promote Equal Opportunities within the scheme.
- Liaise with partners and stakeholders to develop meaningful and sustainable community engagement.
- Provide cover for other Independent Living Co-ordinators and/or Scheme Managers at other Hedyn Extra Care and/or sheltered housing schemes as required by the Specialist Housing Lead or Manager.
- Act upon safeguarding concerns immediately and in compliance with the safeguarding policy and procedure.
- Attend training identified as essential to the Independent Living Co-ordinator role.
- Attend relevant meetings as and when required.
- Act in compliance with GDPR and data protection laws, respecting confidentiality and privacy.
- Ensure that safe working practices and procedures are adopted at all times.
- Have a commitment to the organisation's pledge on Equity, Diversity, and Inclusion.
- Keep abreast of all relevant legislation and good practice issues relating to the role.
- Lead or participate in assigned projects, establish and maintain professional boundaries.
