Duties & Responsibilities
- Assigning appropriate carers who match the preferences of clients
- Managing client relationships
- Ensuring your team documents and maintains clear and accurate records of care given
- Managing a care team, providing consistent support and regular feedback on all aspects of their role
- Following the 'outcomes' model of care, where specific goals and desired outcomes are planned with the input from the client at the beginning of a care package
- Delivering care with these outcomes in mind, with regular feedback from carers essential in monitoring the progress of the client
