About the Service: HVPH is a busy private hospital that provides care to inpatient Rehabilitation, Medical and Surgical patients, as well as providing Day Surgery
and Outpatient Rehabilitation services.
About the Position: As the Social Worker, you will be required to work with inpatients and outpatients from the Rehab, Medical and Surgical Units to provide professional social work service. The service provides a high quality, evidence-based clinical service consisting of assessment, care co-ordination, intervention, liaison, advocacy and review for patients and their family / carers referred to the service.The caseloads are varied and can include neurological, orthopedics, amputee, chronic illness, and palliative care.
The available position offers a Social Worker the opportunity to be part of a friendly multidisciplinary team and work within the Social Work team.
About You: The successful candidate will have:
- An approved Degree in Social Work that includes eligibility for membership with the AASW;
- Police and Working with Children Checks;
- Hospital experience;
- Demonstrated effective verbal, written and interpersonal communication skills including the ability to adapt communication style to suit different audiences;
- Demonstrated conflict resolution skills;
- Ability to prioritise caseload and work within timeframes;
- Demonstrated ability to effectively manage both clinical workload and administrative duties;
- Demonstrated ability to work as a member of a multidisciplinary team;
- Demonstrated commitment to professional development and continuous improvement;
- Knowledge on a variety of health related conditions;
- Ensure patient/consumer care is delivered within professional, organisational, legal and ethical boundaries and reflects evidence based, best practice knowledge;
- Knowledge of local community support services;
- Knowledge of aged care services ;
- Intermediate to advanced computer skills.
