As our services continue to grow, we are looking for a Clinical Homecare Nurse specialising in Oncology and Systemic Anti-Cancer Therapy (SACT) to join our nursing team.

About The Role

This role is flexible and multi-modal, combining homecare visits, clinic delivery and virtual patient support, depending on patient and service needs.

Travel across the East Midlands region will be required.

You will be responsible for delivering safe, effective and evidence-based clinical care in line with professional standards, company policies and regulatory requirements.

Working independently while remaining connected to a wider multidisciplinary team, you will support patients throughout their treatment journey and ensure the highest standards of care are maintained at all times.

This role offers the opportunity to build meaningful relationships with patients while working across a variety of service delivery models.

The successful candidate will be responsible for providing outstanding patient care, promoting positive outcomes and ensuring high standards of clinical governance are consistently achieved.

  • Main Responsibilities
  • Assessing, Planning and Delivering High-Quality Care
  • Manage a caseload of patients with varying conditions and treatment needs
  • Conduct comprehensive patient assessments to determine care requirements
  • Develop and maintain individualised care plans
  • Review and update care plans according to patient needs
  • Administer medications, treatments and therapies as prescribed
  • Deliver oncology and SACT treatments in accordance with approved protocols
  • Monitor patient progress, vital signs and response to treatment
  • Demonstrate sound clinical judgement and risk-based decision making
  • Identify, manage and elevate clinical concerns appropriately
  • Respond confidently to emergencies or sudden deterioration in a patient's condition
  • Support patients experiencing treatment side effects or complications
  • Maintain patient dignity, privacy and independence
  • Work collaboratively with healthcare professionals to deliver comprehensive care
  • Maintain professional competence through ongoing learning and development
  • Recognise personal limitations and seek support where appropriate
  • Participate in on-call services where applicable
  • Educating and Empowering Patients and Caregivers
  • Provide education on disease, treatment and medication management
  • Answer patient and caregiver questions regarding treatment and care
  • Ensure patients and families feel confident managing medications safely
  • Support patients to better understand their treatment journey
  • Encourage independence and self-management where appropriate
  • Assess home environments for safety risks and make recommendations
  • Teach patients and carers to recognise signs of complications or adverse effects
  • Provide emotional support and reassurance throughout treatment
  • Advocate for patients and signpost additional support services where required
  • Clinical Governance and Compliance
  • Adhere to organisational policies, procedures and professional standards
  • Practice in accordance with the NMC Code
  • Report incidents, near misses, adverse events and safeguarding concerns
  • Maintain accurate and timely patient records
  • Participate in quality audits and service reviews
  • Implement actions arising from audit findings where appropriate
  • Carry out risk assessments and implement risk reduction measures
  • Support continuous improvement initiatives
  • Ensure compliance with regulatory, governance and confidentiality requirements
  • Promote a culture of accountability, learning and clinical excellence
  • Communication and Collaboration
  • Maintain effective communication with patients and families
  • Work closely with consultants, GPs and multidisciplinary teams
  • Communicate patient status and clinical information effectively
  • Explain complex medical information in a clear and understandable way
  • Build positive professional relationships with internal and external stakeholders
  • Work alongside Nursing Service Managers to identify risks and opportunities
  • Support service development and continuous improvement
  • Mentor, train and support colleagues where appropriate

What Experience and Skills Are We Looking For?

  • Active Registered Nurse (RN) with current NMC registration
  • Independent Prescriber
  • Valid SACT Passport up to date with at least 6 months remaining.
  • Clinical experience in Oncology
  • IV therapy experience
  • Proficiency in patient assessment, medication administration and clinical procedures
  • Ability to work independently and manage competing priorities
  • Excellent verbal and written communication skills
  • Ability to build rapport and trust with patients and families
  • Knowledge of safety procedures within homecare and clinic environments
  • Accurate and timely documentation skills
  • Flexible and adaptable approach to work
  • Ability to educate patients and caregivers effectively
  • Full UK driving licence
