Job Purpose
To oversee and support the care delivery team in the community, ensuring high standards of care are maintained and that clients' needs are met in accordance with their care plans. The role bridges office-based management with front-line care delivery, ensuring smooth service provision even in challenging situations.
Responsibilities
- Care Coordination & Quality Monitoring
- Conduct regular spot checks, supervisions, and quality monitoring visits to ensure compliance with CQC regulations and company standards.
- Carry out initial client assessments, risk assessments, and care planning in line with regulatory requirements.
- Review and update care plans and risk assessments as needed.
- Liaise with healthcare professionals, service users, and family members to ensure coordinated care.
- Support to Carers
- Provide hands-on training, mentoring, and shadowing to new and existing carers.
- Act as a point of contact and support for carers in the field.
- Ensure carers understand and follow their schedules and care plans.
- Investigate and report incidents, complaints, or safeguarding concerns appropriately.
- On-Call Commitment
- Participate in the on-call rota, providing out-of-hours support to staff and clients.
- Respond promptly and effectively to emergency situations or calls from clients/carers during on-call hours.
- Coordinate emergency cover or support when care staff are unavailable.
- Shift Cover Responsibilities
- Cover care visits personally in the event of last-minute cancellations, emergencies, or staff shortages, including early mornings, evenings, and weekends.
- Maintain readiness to step in as a carer when necessary to avoid service disruption.
- Compliance and Record Keeping
- Ensure all documentation, including MAR charts and daily notes, are completed accurately and promptly.
- Maintain compliance with GDPR, safeguarding policies, and confidentiality.
- Participate in audits and contribute to continuous service improvement.
Person Specification
Essential
- NVQ Level 2 in Health & Social Care or equivalent.
- Minimum 2 yearsβ experience in domiciliary/home care, including supervisory or senior care responsibilities.
- Good understanding of CQC requirements and person-centred care.
- Full driving licence and access to a vehicle.
- Flexibility to work varied hours including evenings, weekends, and participate in on-call duties.
Desirable
- Experience with care planning software.
- Training qualification (Train the Trainer) or willingness to achieve it.
- First aid, medication administration, and manual handling certification.
