Outreach is a social care provider with over 28 years of experience in caring for people in their own homes. We are looking for honest and caring people to join our existing team. This role requires you to have a full enhanced police check, which is paid for by the company. The role is office and community based covering the Southport area. The team leader role requires you to work with the care manager to ensure high standards of care are delivered to our clients at all times.

You will monitor the performance of care staff and offer help and support to them when needed, to ensure high standards are maintained. You will work alongside staff delivering care to clients as part of the team leader role.

Main Duties:

  • Risk assessments - plan visits, complete digital records and update client files.
  • Care Plans - carry out annual reviews, complete digital records and update client files.
  • Client reviews - carry out bi-annual reviews for clients.
  • Staff Observations - carry out bi-annual reviews for staff, deal or report issues or training requirements.
  • Work with management team to keep clients and staff safe and report or deal with issues promptly.
  • Delivering high quality person-centred care to our clients.
  • Take potential new staff on shadowing and training shifts to ensure suitability.

Shifts:

  • Full time guaranteed hours contract
  • Mornings: 7am - 2pm
  • Evenings: 4pm - 11pm
  • Alternate weekend availability essential
  • Outreach can be flexible on shifts depending on skills and experience - willing to work around your availability

Pay:

  • Base - Β£14 per hour
  • Weekends / Evenings - Β£16 per hour
  • Travel time and Mileage at 45p

Requirements:

  • Valid driving licence and own car
  • Female and male applicants are welcome
  • Minimum 1 year experience in a social care environment
  • Willing to undergo an NVQ in Health & Social Care
  • Please be aware we do not offer sponsorship
