The Role
- Management of clients and responsibility for their ongoing care and support with a focus on person centred care.
- Undertake competency assessments and/or onsite support of Care Professionals in the field as needed to support clients.
- Meet and carry out assessments with new clients, and complete all paperwork as required.
- Achieve agreed KPI’s for Care Professional and client touchpoints.
- Create, update and audit client care plans.
- Maintain accurate client records on Home Instead’s systems.
- Conduct client and Care Professional introductions.
- Build and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care.
- Carry out client Service Reviews, Quality Assurance visits and address any resulting actions.
- If required, support, mentor and supervise Care Professionals in relation to their client’s needs and outcomes.
- Provide cover for absence of others in the care and care professional supervisory team.
- Participate in on-call duties (weekday evenings and occasional weekends).
- Ensure compliance with Home Instead’s Equality, Diversity and Equal Opportunities Policy, in respect of employment and service delivery.
- Carry out any other duties deemed necessary for the successful operation of the business.
- Delivery of care when required.
