Role Overview
We are looking for a highly motivated and enthusiastic Care Coordinator to join our team to help start up our franchised office covering Tonbridge, Tunbridge Wells, Paddock Wood and surrounding villages. As the Care Coordinator you will support the registered Manager in the day-to-day management of the service. You will manage the weekly rotas and staffing issues that arise, ensuring compliance with all statutory and regulatory requirements. You will play a vital role in ensuring the well-being and safety of clients while promoting a compassionate and nurturing environment. You will be expected to cover emergency calls alongside the management and office team and take your turn at the on-call service.
Key Responsibilities
- Coordinate, schedule, and match care visits to ensure continuity and person-centred support.
- Communicate effectively with clients, families, and care professionals.
- Support with care assessments, care planning, and reviews.
- Maintain accurate electronic records and ensure compliance with CQC standards.
- Provide day-to-day support to carers, helping them feel valued and equipped to deliver outstanding care.
- Participate in on-call duties on a rota basis.
Ideal Skills Set and Experience
- NVQ Level 3 in Health and Social Care or be willing to work towards the qualification or equivalent and a willingness to progress further.
- At least 1 year experience as a care coordinator in a domiciliary care service.
- Strong administration and rota planning skills.
- Excellent communication, Information technology skills and time management skills.
- Flexibility to maintain the rota and cover calls as and when an emergency arises with our dedicated carers.
- Excellent knowledge and understanding of compliance and legislative requirements of the Care Standards regulations.
- Strong skills in conducting care assessment and care planning using Access.
- Good working knowledge of IT Systems with experience of Microsoft Office or Google Suite.
- Excellent communication, leadership and organisational skills.
- Essential to work under own initiative and be able to problem solve.
- Ability and confidence to build and maintain relationships with clients, team members, families and other multi-disciplinary agencies.
Other Job Requirements
- Must have Right to Work in the UK.
- Enhanced DBS disclosure.
- Full and valid UK Driver's license and access to a Vehicle.
