We are currently recruiting Support Workers to join a new package of care in the Newport area. You will be providing one-to-one and two-to-one support to an individual with mental health needs and a physical disability, within his own home and out in the community. This is a rewarding role focused on delivering person-centred, values-based care, supporting independence, dignity, safety, and meaningful daily routines.
Key Responsibilities:
- Supporting with all activities of daily living
- Promoting independence, dignity, and personal choice
- Monitoring health, wellbeing, and emotional needs
- Supporting access to the community and meaningful activities
- Maintaining accurate daily records and documentation
- Following individual care plans and risk assessments
- Adhering to safeguarding policies and procedures
- Supporting faith-based routines respectfully and appropriately
Shifts: Full time and part time contracts are available - 12 to 48 hours per week
Patterns:
- Day shifts: 8am – 8pm
- Waking nights: 8pm – 8am
- Sleep-in shifts
- Day workers must be available for sleep ins
