PAY: £28,000-£30,000 (Depending on experience) + £100 per on-call weekend (£1200 annually)
RESPONSIBLE TO: Deputy Care Manager
JOB PURPOSE
To manage all aspects of future and day-to-day planning, to ensure smooth care service delivery.
WORK HOURS
40 hours per week plus on-call duties as per the office rota (one weekend in four on-call, one evening/morning per week on call).
MAIN DUTIES
- Optimally allocate CareAngels based on skills, availability, experience and client needs to deliver the best quality care and support to each of our clients.
- Organise CareAngel rotas to minimise travel between clients and ensure guaranteed hours are allocated, where necessary.
- Respond efficiently to day-to-day changes in client care needs and CareAngel availability, communicating professionally to all concerned.
- Accurately record all changes to scheduled visits in a timely manner to ensure clear visibility and full traceability.
- Maintain accurate and professional written communication.
- Monitor all relevant aspects of the care management system.
- Remain hands-on in the field by attending scheduled care visits.
- Undertake on-call duties as per the office colleague rota.
ACTIVITIES
MONTHLY
- Manage holiday and bank holiday planning process.
- Feedback to Operations Manager and Registered Manager regarding concerns or recommendations for process improvements.
WEEKLY
- Future planning of rotas.
- Ensure rotas are planned 4 weeks in advance.
- Maintain trackers to identify upcoming capacity constraints and recruitment needs.
- Ensure guaranteed hour CareAngels have the correct number of hours.
- Complete Return to Work forms for all colleagues on sick leave.
- Welcome new colleagues.
- Meet and greet new colleagues during their training.
- Book shadow visits and communicate with new colleagues.
- Add new CareAngel availability on OTH.
- Other planning tasks.
- Update the Operations Manager on branch care hours capacity.
- Update Recruitment Manager on recruitment needs.
- Update CareAngel availability.
- Manage CareAngel holiday requests.
- Email client rotas.
- Notify CareAngels of finalised rotas.
- Follow up with clients on service suspend.
- On-call duties as per rota.
DAILY
- Monitor One Touch Health to ensure no missed visits, medications or tasks.
- Care Coordination.
- Manage CareAngel sickness and cover unallocated visits.
- Respond to rota change requests e.g. cancellations, time changes, ad-hoc extra visits.
- Communicate and confirm rota changes with clients and CareAngels.
- Colleague profile changes.
- Amend digital tasks and medication on OTH as required.
- New client enquiries.
- Take new client enquiries and complete enquiry form.
- Send brochures by post/email.
- Book assessments with Registered Manager and/or Client Advisor.
- Follow up accordingly on all enquiries as required.
Other
- Support Office with non-urgent tasks e.g. placing orders for PPE, stationery etc., organising post, add CareAngel contact details to on-call phone.
- Work in accordance with the policies, procedures and philosophy of Guardian Angel Carers.
- Contribute to a healthy and safe working environment.
- Any other tasks as delegated by the Business Owners.
