Primary Purpose of the role:

To be responsible for delivering high quality, person-centered care and support for clients, including elderly people, those with disabilities and health conditions and other vulnerable people, to help them live as independently and safely as possible in their homes and the community.

Core Responsibilities and Tasks:

  • To enable clients to gain independence, increase confidence and promote health and rehabilitation including with the following: personal care and hygiene, continence management, pressure sore prevention, medication support, monitoring diet and fluid intake, promoting diet and fluid intake, daily living/homecare tasks and assistance and social engagement.
  • To deliver care and support to clients on a day-to-day basis in line with agreed care packages, an understanding of their routines and preferences and to the highest standards of choice, dignity and respect.
  • To provide companionship and emotional support to clients, fostering positive and trusted relationships with the client and their family/relevant others.
  • To ensure that cleanliness and safety of the clients’ home environment, proactively reporting any issues or concerns to service management.
  • To follow all health and safety procedures including use of infection prevention and control protocols.
  • To liaise with other professionals e.g. G.Ps, District Nurses, Social Workers, Pharmacists, chiropodists, to support the delivery and continuity of appropriate care and support to clients.
  • To develop and maintain positive relationships with clients, family members, carers and/or friends involved in their daily lives, care and support.
  • To assist with the review of home care services and client reviews including attending meetings with Care Managers as required.
  • To comply with all relevant standards, policies and procedures of the company ensuring the highest quality and safety of service at all times.
  • To keep timely and accurate records of client care, dietary and fluid monitoring records, medication records and employee expenses, time and mileage sheets.
  • To maintain confidentiality at all times in line with Carers and Companions policies and accurate and timely record keeping compliant with relevant data protection and confidentiality requirements.
  • To identify improvements to the service including sourcing and reporting feedback from clients, their families and other relevant parties.
  • To keep up to date with best practice, new developments and regulatory changes to health and social care.
  • To be a supportive member of the team, contributing positively to the continuous improvement of the service.
  • Other duties commensurate with the role as agreed.
