The Care Coordinator will support the effective day-to-day running of supported living and domiciliary care services. The role involves coordinating care delivery, supporting care staff, and ensuring high-quality, person-centred care is delivered in line with company policies, CQC standards, and individual care plans.
Key Responsibilities
- Coordinate and oversee supported living and domiciliary care packages
- Allocate care staff to service users, ensuring continuity and quality of care
- Act as a key point of contact for service users, families, care staff, and external professionals
- Support care staff with day-to-day issues and escalate concerns where appropriate
- Participate in on-call duties to respond to out-of-hours emergencies and staffing issues
- Assist with care plan implementation, reviews, and updates
- Monitor service delivery to ensure compliance with CQC regulations and company policies
- Support the induction and ongoing development of care staff
- Report safeguarding concerns and incidents in line with procedures
- Maintain accurate records related to care delivery and staffing
Person Specification
Essential
- Passion for delivering high-quality care
