- Conduct detailed assessments within Adult Social Care, tailoring your approach to each individual's needs
- Oversee a caseload involving adults requiring equipment, adaptation, and moving and handling support
- Deliver reablement-focused strategies to aid service user independence
- Engage in reviews aimed at reducing care packages through strategic DHC to SHC evaluations
- Recommend equipment and create adaptation solutions customized for each user's living environment
- Partner closely with care coordinators, service users, and families to devise strengths-based, person-centred care plans
- Ensure the highest standards of safety through comprehensive moving and handling assessments, implementing safe practices
- Coordinate with multidisciplinary teams and partner agencies to deliver holistic care
- Maintain precise and thorough case records and reports
