What you will do:

• - Assess and treat predominantly older adult patients while delivering community-based rehabilitation programmes
• - Support patients to maximise independence and functional outcomes, ensuring high-quality care
• - Complete comprehensive physiotherapy assessments and develop as well as review person-centred treatment plans
• - Work effectively within multidisciplinary community teams to facilitate safe discharge and ongoing rehabilitation
• - Maintain accurate clinical records and documentation
• - Provide specialist advice to patients, carers, and healthcare professionals
• - Manage an independent community caseload ensuring patient-centred care and effective treatment strategies
• - Community rehabilitation setting focused on older adults
• - Access to your own vehicle is necessary for community work, providing the freedom to manage your caseload across different locations

Requirements:

• - Degree or Diploma in Physiotherapy
• - HCPC registration
• - Band 6 Physiotherapy experience with particular experience working with older adults
• - Previous community rehabilitation experience with strong clinical assessment and reasoning skills
• - Full UK driving licence with access to a vehicle for community visits
• - Experience with NHS Scotland
• - Community physiotherapy experience within multidisciplinary rehabilitation teams is preferred
