What you will do:

• Review existing care packages and support arrangements to ensure optimal outcomes.
• Reassess previous evaluations and compile supporting information for continuity of care.
• Identify and promote opportunities to increase a client's independence and functional capabilities.
• Provide well-rounded recommendations to enhance care and support plans.
• Conduct further Occupational Therapy assessments as required to inform decisions.
• Assess functional abilities and daily living skills to tailor interventions.
• Complete necessary moving and handling assessments for safe and effective practices.
• Foster effective resource utilisation through evidence-based methods.
• Work collaboratively with families, practitioners, and partner organisations for a comprehensive approach.
• Maintain thorough records, reports, and recommendations for continuous service improvement.

Requirements:

• Degree or Diploma in Occupational Therapy and HCPC registration.
• Experience working with children and young individuals with disabilities.
• Proficient in functional assessment and moving and handling assessment experience.
• Knowledgeable in independence-focused practice.
• Capable of managing a varied and dynamic caseload.
• Strong skills in communication, assessment, and report writing.
• Experience within Children's Disability Services and reviewing care packages.
• Contributions to service improvement initiatives and working within multidisciplinary teams.
