Primary Care Organisation (Somerset) GP practices within a Primary Care Network in Yeovil, Somerset
Permanent | Part-time | 30 hours per week
Pay - up to £43,000 per annum

Job summary
An established primary care organisation is seeking an experienced Practice Nurse with a background in chronic disease management to work across multiple GP practices within a Yeovil-based Primary Care Network. The role focuses on delivering high-quality, patient-centred care and supporting long-term condition management within the community.
Main duties
- Manage and deliver long-term condition clinics
- Diagnose, monitor, and review patients with chronic diseases
- Develop, implement, and update personalised care plans
- Complete QoF and enhanced service annual reviews using clinical templates
- Provide same-day acute patient reviews when required
- Process pathology results, workflows, and support clinical audits
- Refer patients to multidisciplinary services including social prescribing and wellbeing teams
- Supervise, support, and train other members of the clinical team
Person specification
Essential
- Registered Nurse with current NMC registration
- Qualifications and experience in chronic disease management
- Minimum of 2 years' recent experience managing patients with long-term conditions
- Strong communication, clinical decision-making, and care coordination skills
- Knowledge of local and national health policy
Desirable
- Mentor or teaching qualification
- Clinical supervision training and experience
- Knowledge of EMIS, SystmOne, and Ardens
- Non-medical prescribing and advanced clinical skills
