A GP surgery is seeking an experienced and compassionate Qualified Practice Nurse to deliver high-quality, evidence-based care in Primary Care. The role focuses on chronic disease management, preventative healthcare, and health promotion, working flexibly as part of a multidisciplinary team. Postholders are expected to demonstrate:
