About the Role
We are seeking an enthusiastic and motivated Practice Nurse to join a supportive Primary Care team. This is a permanent opportunity, with flexibility to accommodate either full-time or part-time working patterns.
The successful candidate will be an integral part of the clinical team, providing high-quality nursing care and supporting patients in the management of both acute and long-term health conditions.
Key Responsibilities
- Chronic disease management (e.g. diabetes, COPD, asthma, hypertension)
- Childhood and adult immunisations
- Cervical cytology (smears)
- Wound management and dressings
- Health screening and health promotion
- Assisting with QOF targets and audits
- Contributing to the smooth running of the practice clinical team
Requirements
- NMC registered nurse with Primary Care/Practice Nursing experience
- Competency in treatment room skills and chronic disease management
- Evidence of continued professional development
- Excellent communication and teamwork skills
