Practice Nurse – Primary Care (Permanent)

About the Role

We are seeking an enthusiastic and motivated Practice Nurse to join a supportive Primary Care team. This is a permanent opportunity, with flexibility to accommodate either full-time or part-time working patterns.

The successful candidate will be an integral part of the clinical team, providing high-quality nursing care and supporting patients in the management of both acute and long-term health conditions.

Key Responsibilities

  • Chronic disease management (e.g. diabetes, COPD, asthma, hypertension)
  • Childhood and adult immunisations
  • Cervical cytology (smears)
  • Wound management and dressings
  • Health screening and health promotion
  • Assisting with QOF targets and audits
  • Contributing to the smooth running of the practice clinical team

Requirements

  • NMC registered nurse with Primary Care/Practice Nursing experience
  • Competency in treatment room skills and chronic disease management
  • Evidence of continued professional development
  • Excellent communication and teamwork skills
  • A proactive, flexible and patient-centred approach
