About the Role

We are seeking an enthusiastic and motivated Practice Nurse to join a supportive and welcoming Primary Care team. This is a permanent opportunity within a close-knit practice that is part of a wider group of GP surgeries who work collaboratively to improve both staff and patient experiences.

The successful candidate will play an integral role within the clinical team, delivering high-quality nursing care and supporting patients in the management of both acute and long-term conditions.

Key Responsibilities

  • Chronic disease management (e.g. diabetes, COPD, asthma, hypertension)
  • Childhood and adult immunisations
  • Cervical cytology (smears)
  • Wound care and dressings
  • Health screening and promotion
  • Supporting the achievement of QOF targets and audits
  • Contributing to the smooth and efficient running of the clinical team

Requirements

  • NMC registered nurse with experience in Primary Care or Practice Nursing
  • Competency in treatment room skills and chronic disease management
  • Evidence of continued professional development
  • Excellent communication and teamwork skills
  • A proactive, flexible, and patient-centred approach
