Responsibilities
- Conduct remote consultations (telephone/video) with patients
- Manage and review patients with long-term conditions (e.g. diabetes, asthma, COPD, hypertension)
- Deliver evidence-based care and support self-management strategies
- Perform care planning and medication reviews where appropriate
- Maintain accurate clinical records using SystmOne
- Work collaboratively with GPs, HCAs, and the wider multidisciplinary team
- Support QOF targets and chronic disease management programmes
Qualifications
- Registered Nurse (RGN) with valid NMC registration
- Experience working in primary care / GP surgery
- Strong background in long-term condition management
- Proficiency with SystmOne (essential)
- Excellent communication skills, particularly in remote consultations
- Ability to work independently and manage workload effectively
- Chronic disease diplomas or relevant certifications
- Experience in remote/telehealth environments
- Have your own laptop with S1 set up
- Prescribing qualification (desirable but not essential)
