We are seeking an experienced Remote Clinical Pharmacist to support two Primary Care Networks (PCNs) on a part-time basis. This is an excellent opportunity for a motivated pharmacist with strong primary care experience to work flexibly while contributing to high-quality patient care.

Key Responsibilities

  • Deliver remote clinical pharmacy services across two PCNs
  • Manage and support patients with long-term conditions
  • Contribute to QOF achievement and medicines optimisation
  • Undertake prescribing activities in line with best practice and PCN priorities
  • Work collaboratively with GP practices and the wider multidisciplinary team

Essential Requirements

  • Registered Clinical Pharmacist with independent prescribing qualification
  • Previous GP surgery / primary care experience
  • Proven experience with QOF and long-term condition management
  • Ability to work remotely with minimal supervision
  • Own laptop with EMIS or SystmOne already set up
  • Strong communication and organisational skills

Role Details

  • Fully remote position
  • 3 days per week
  • Supporting two PCNs
  • 6-month fixed-term contract
  • Competitive hourly rate of 30-35, depending on experience
