We are seeking an experienced Remote Clinical Pharmacist to support two Primary Care Networks (PCNs) on a part-time basis. This is an excellent opportunity for a motivated pharmacist with strong primary care experience to work flexibly while contributing to high-quality patient care.
Key Responsibilities
- Deliver remote clinical pharmacy services across two PCNs
- Manage and support patients with long-term conditions
- Contribute to QOF achievement and medicines optimisation
- Undertake prescribing activities in line with best practice and PCN priorities
- Work collaboratively with GP practices and the wider multidisciplinary team
Essential Requirements
- Registered Clinical Pharmacist with independent prescribing qualification
- Previous GP surgery / primary care experience
- Proven experience with QOF and long-term condition management
- Ability to work remotely with minimal supervision
- Own laptop with EMIS or SystmOne already set up
- Strong communication and organisational skills
Role Details
- Fully remote position
- 3 days per week

- 6-month fixed-term contract
- Competitive hourly rate of 30-35, depending on experience
