Diabetes Specialist Practice Nurse

You'll lead on diabetes management and patient education, working closely with our multidisciplinary team to deliver high-quality, patient-centred care. Key Responsibilities:

• Provide expert diabetes care and support for patients
• Run nurse-led diabetes clinics and support lifestyle interventions
• Collaborate with GPs, pharmacists, and healthcare assistants
• Contribute to service improvement and QOF outcomes

What We’re Looking For:

• NMC-registered Nurse with post-registration experience
• Relevant qualification or strong interest in diabetes care
• Excellent communication, clinical, and organisational skills
• Passion for health promotion and patient empowerment
