Clinical Pharmacist – General Practice
Location: Reading
Type: Permanent, Full Time (37.5 hours, some flexibility available)
An established GP surgery in the Reading area is seeking a motivated Clinical Pharmacist with experience in General Practice to join their friendly, multi-disciplinary team. This role is pivotal in delivering safe, effective, and patient-centred Clinical pharmacist services within primary care.
What you’ll be doing:
- Managing repeat prescriptions, acute prescription requests, medicines reconciliation and transfers of care
- Providing structured medication reviews and telephone clinics to support patients with queries about their medicines
- Supporting safer prescribing systems, QOF and enhanced services, audits, PINCER, and the Prescribing Quality Scheme
- Building strong links with community pharmacies, PCN pharmacy teams, and care homes to improve medicines safety
- Working as part of a supportive team to ensure high standards of patient care
What we’re looking for:
- Must have experience working in a GP surgery
- CPPE training completed
- Independent Prescriber qualification (essential)
- Sound clinical knowledge with a strong understanding of prescribing and medication reviews
- A collaborative approach with the ability to work autonomously within a multi-disciplinary team
