Clinical Pharmacist – New Malden PCN

Salary: Up to Β£59,000 per annum | Full-time hours

A forward-thinking Primary Care Network in New Malden is seeking a Clinical Pharmacist to join their team on a permanent, salaried basis. Key responsibilities

  • Conduct patient-facing medication reviews and long-term condition clinics
  • Support care homes, domiciliary visits, and post-hospital discharge medicines reconciliation
  • Provide telephone support and advice for patients with medicines queries

What we’re looking for

  • GPhC registered Pharmacist (Independent Prescriber desirable)
