Healthier West Wirral PCN seeks a dedicated, experienced GP to join our team, working 5 sessions per week split between:
- Acute Visiting Service (AVS) – home visits
- PCN GP Hub – same‑day appointments across all five member practices booked into cross‑organisational slots
This is an exciting opportunity to work in a centralised hub model while supporting innovative primary care delivery and improving patient access across the network.
Main Duties
- Deliver home visits and acute care via the AVS to housebound and care home patients
- Provide same‑day appointments from the PCN GP Hub for patients across all five practices
- Assess, diagnose, and manage a wide range of acute and urgent conditions
- Work closely with the multidisciplinary PCN team, including nurses, physician associates, ACPs, pharmacists, and paramedics
- Contribute to improving access to care, reducing waiting times, and supporting clinical capacity across the network
- Maintain accurate patient records and adhere to PCN clinical governance standards
- Participate in team meetings, learning, and service improvement initiatives
- Apply clinical judgment to determine the scope of patients seen within the AVS and Hub framework

Responsibilities
- Acute Visiting Service (AVS) – Undertake home visits for patients in need of medical attention, provide comprehensive and timely care, collaborate with practice staff and community health teams, ensure accurate documentation, triage and treat patients, manage referrals to secondary care, maintain clinical standards, and contribute to audits and service improvement.
- PCN GP Hub – Deliver high‑quality general practice care across the network, including face‑to‑face same‑day consultations, prescription management, administrative duties, long‑term condition management, clinical investigations, referrals, collaboration with multidisciplinary teams, and participation in clinical meetings.
Person Specification – Experience
Essential
- Qualified GP, listed on NHS England’s register of medical practitioners.
- Full registration with the General Medical Council (GMC) on the General Practitioner Register.
- Compliance with all regulatory requirements, including the NHS Performers List regulations and no suspension from practice.
- Employment terms at least as favourable as those outlined in the Model Terms and Conditions of Service for a salaried GP.
- Legal right to work in the UK and appropriate medical indemnity insurance.
- Enhanced Disclosure and Barring Service (DBS) check completed and valid digital badge for secure system access.
- Willingness to engage in ongoing professional development in line with PCN and ARRS requirements.
Desirable
- Experience working in primary care and/or GP practice.
- Knowledge of Primary Care Networks.
- Excellent interpersonal skills and ability to work effectively within a multidisciplinary team.
- Strong time‑management and independent working skills.
