Job Summary
The role requires availability on a rota basis and occasional Saturday work as part of the Enhanced Access service.
Responsibilities
- Run independent acute care clinics and provide responsive home visits, ensuring patients receive quality care where they feel safest.
- Lead proactive care home ward rounds, improving outcomes and quality of life for residents.
- Manage patients on unplanned or urgent care pathways, applying the NHS Five Safes framework, NICE and local guidance.
- Assess individual patient needs, formulate diagnoses and initiate evidence‑based interventions, developing personalised holistic management plans.
- Conduct diagnostics including ECG and phlebotomy, and support the use of near‑patient testing where appropriate.
- Communicate sensitive information with patients, carers and multidisciplinary teams, using alternative methods as required.
- Maintain accurate, contemporaneous patient records and participate in audit and quality improvement activities.
- Travel across the South Wirral footprint and work physically demanding tasks such as patient examination and lifting.
- Prescribe and review medication within scope of practice, supporting patient adherence and providing medication counselling.
- Identify and refer patients with mental health needs in accordance with the NHS Five Safes for Mental Health.
- Support shared learning across the practice and wider organisation, contributing to team appraisal and professional development.

Healthier South Wirral (HSW) PCN comprises five GP practices: Eastham Group Practice, Sunlight Group Practice, The Orchard Surgery, Civic Medical Centre and Spital Surgery. We focus on delivering proactive, personalised and integrated care across acute clinics, home visits, care home ward rounds, mental health support and preventative programmes.
Person Specification
Qualifications – Essential
- Advanced Nurse Practitioner or HCPC Registered Paramedic
- Degree or diploma level in Paramedicine or equivalent experience
- Full UK driving licence
Qualifications – Desirable
- Non‑medical prescribing competence
Experience – Essential
- Accountability for own roles and other roles in a primary care setting
- Highly developed knowledge of illness and injury presentation
- Clinical governance experience in primary care
- Experience working with frail and elderly patients
- Knowledge of local and national health policy
- Experience managing acute illness presentations
- Current and up‑to‑date professional development portfolio
Experience – Desirable
- Minimum two years post‑registration clinical practice experience
