This is your opportunity to make a real difference to some of the most vulnerable people in our community, while working in a forwardâthinking environment that respects your clinical judgment and supports your growth.Responsibilities Run acute care clinics with autonomy, supported by a team who values your input.Provide responsive home visits, allowing patients to receive quality care where they feel safest.Lead proactive care home ward rounds, improving outcomes and quality of life for residents.Work alongside a multidisciplinary team that includes GPs, pharmacists, social prescribers, and mental health professionals.Drive innovation in care delivery with the support of a PCN that invests in its people and infrastructure.Be available on a rota basis to work within our Enhanced Access service, including occasional Saturday work.Main Duties Deliver care across acute clinics, home visits, and care home ward rounds, contributing to personâspecific models of care such as frailty pathways.Autonomously manage patients with unplanned or urgent healthcare needs and complex longâterm conditions.Assess individual patient needs, formulate diagnoses, and initiate appropriate, evidenceâbased interventions.Develop personalised, holistic management plans tailored to each patient\âs circumstances.Ensure delivery of consistent, highâquality, personâcentred care consistent with NSF, NICE guidance and ICB policies.Collaborate with other clinical teams to improve quality of health care and support shared learning.About Us Healthier South Wirral consists of 5 GP practices: Eastham Group Practice, Sunlight Group Practice, The Orchard Surgery, Civic Medical Centre, and Spital Surgery. We focus on delivering proactive, personalised, and integrated care , with services spanning acute clinics, home visits, care home ward rounds, mental health support, and prevention programmes .
