Health Hero is recruiting an Advanced/Enhanced Clinical Practitioner for Care Coordination & Out of Hours, delivering triage, assessment and prescribing across Bath, Swindon and Wiltshire.

You will work across Care Coordination and urgent care, on a permanent basis, with flexible rota options.

You will join a team that supports ambulance services, primary and secondary care to optimise patient pathways and reduce unnecessary hospital visits.
