HealthHero Integrated Care in the Bath, Swindon and Wiltshire area is seeking independent, autonomous Prescribing Clinicians to support our Urgent Care service during out-of-hours. You will work from Chippenham HQ or Primary Care Centres across the locality, with shifts including face-to-face, home visits, and regional triage.
As an Advanced Clinical Practitioner, you will assess, diagnose, treat and refer patients, triage calls, and support the Care Coordination Service.
