Responsibilities
- Conduct initial physical therapy evaluations and develop individualized treatment plans for residents in the skilled nursing setting.
- Implement therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
- Reassess patient progress, adjust treatment plans, and document outcomes.
- Collaborate with the interdisciplinary care team; communicate with physicians, therapists, and families as needed.
- Maintain accurate, timely documentation in patient records, including treatment notes and progress notes.
- Ensure safety, comply with regulatory requirements and facility policies, and provide patient and family education.
Requirements
- Active Physical Therapist license to practice in Massachusetts.
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
- Willingness to commit to a 13-week travel assignment.
- Ability to work on-site in a skilled nursing facility setting in Leeds, MA.
- Experience in skilled nursing facility or geriatric rehabilitation settings preferred.
- BLS/CPR certification preferred.
