Responsibilities

  • Conduct initial physical therapy evaluations and develop individualized treatment plans for residents in the skilled nursing setting.
  • Implement therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
  • Reassess patient progress, adjust treatment plans, and document outcomes.
  • Collaborate with the interdisciplinary care team; communicate with physicians, therapists, and families as needed.
  • Maintain accurate, timely documentation in patient records, including treatment notes and progress notes.
  • Ensure safety, comply with regulatory requirements and facility policies, and provide patient and family education.

Requirements

  • Active Physical Therapist license to practice in Massachusetts.
  • Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
  • Willingness to commit to a 13-week travel assignment.
  • Ability to work on-site in a skilled nursing facility setting in Leeds, MA.
  • Experience in skilled nursing facility or geriatric rehabilitation settings preferred.
  • BLS/CPR certification preferred.
