Responsibilities:
- Conduct comprehensive speech, language, voice, and cognitive-communication assessments for residents in a long-term care setting
- Develop, implement, and monitor individualized treatment plans to address identified needs
- Provide direct therapy and document progress notes in the medical record; maintain accurate, timely documentation
- Collaborate with an interdisciplinary team including nursing staff, physicians, occupational and physical therapy, and social work to coordinate care
- Educate residents and families on communication strategies, therapy goals, and discharge planning considerations
- Participate in care planning and support transitions between levels of care as needed
Requirements:
- Active Speech-Language Pathologist license in New Hampshire or eligibility for licensure in NH
