Central London Healthcare Ci C in Westminster is seeking a Care Navigator on a fixed term contract until 31 March 2027.

The role is 37.5 hours per week with a salary of £31,750 per annum, and a hybrid/office-based arrangement at our Westminster hub.

You will support GPs through care coordination for vulnerable patients, manage care plans, work with multiple partners and take part in MDTs.

Strong IT and communication skills, plus experience with Systm One, are important.
