Job Overview
In Healthcare Central London (Ci C), a GP Federation covering 30 General Practices in Central London, we are seeking a Care Navigator to support GPs in providing care coordination for vulnerable and complex patients.
Key Responsibilities
- Support patients to develop and review Care Plans in line with a personalised care approach.
- Provide intelligent tasking and patient referrals, completing Care Plans and delivering targeted public health messages.
- Serve as primary point of contact via telephone, Surgery Connect telephony system and email.
- Use Systm One to record all patient interactions, maintain accurate records, and comply with GDPR.
- Participate in Regular MDTs and ICTs, chairing where required.
- Coordinate and navigate patients and carers across health and social care services.
- Refer patients to third‑sector providers and identify unaddressed needs.
- Identify potential safety concerns, inform relevant professionals.
- Work collaboratively with the Care Navigator team to achieve organisation and personal targets.
- Support the Complex Case Management service and contribute to administrative tasks within the GP Federation.
Qualifications & Experience
- GSCE Passes at Grade C or above or equivalent experience.
- Excellent written and verbal English, numeracy and keyboard skills.
- Strong customer service, administrative and organisational skills.
- IT literate; experience using Systm One.
- Knowledge of Medical Terminology and the Health & Social Care system.
- Strategic understanding of NHS Primary Care services.
- Practical knowledge of healthcare improvement methods and practices.
- Flexible and adaptable, able to work across changing needs.
- Commitment to personal development and use of own judgement, resourcefulness, and local knowledge.
- Experience creating Personalised Care & Support Plans; coaching approach.
- Gauge decisions according to situation and adhere to PCN policies.
- Availability to cover annual leave and sickness.
