Job Responsibilities

  • Provide clinical assessment and management of patients on anticoagulation therapy (including initiation, monitoring, and dose adjustment).
  • Prescribe and optimize anticoagulation treatment in line with local and national guidelines.
  • Conduct patient consultations, review INR results, and ensure timely follow-up.
  • Maintain accurate clinical records and contribute to audit and quality improvement initiatives.
  • Collaborate with GPs, nurses, and other healthcare professionals to ensure seamless patient care.
  • Support education and counselling for patients regarding anticoagulation therapy and adherence.

Qualifications

  • Qualified Pharmacist with independent prescribing qualification.
  • Current registration with the General Pharmaceutical Council (GPhC).
  • Experience in anticoagulation management or relevant clinical areas.
  • Strong communication and organisational skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Previous experience in primary care or community‑based anticoagulation services.
  • Familiarity with clinical systems (e.g., EMIS, INR Star).
