What you will do
- Conduct comprehensive assessments and reviews of need under the Care Act 2014.
- Provide expert advice, techniques, equipment, and adaptations to support service users.
- Facilitate access to universal and voluntary services that enhance user independence.
- Collaborate effectively with colleagues across various teams including the Sensory and Telecare Teams.
- Engage in interdisciplinary team working to cultivate and maintain strong relationships with service users and carers.
