Overview
Registered General Nurse (RGN) with a strong background in complex care to join Health Now LTD. This part-time, permanent role involves delivering patient-centred care in service users\\\β own homes, providing clinical leadership, and ensuring safe, effective and compassionate care.
The ideal candidate will have experience managing patients with multiple comorbidities and a range of complex care needs, and will uphold NMC standards and best clinical practice.
Key Responsibilities
- Conduct thorough staff competency assessments and create tailored development plans to support clinical growth and maintain high care standards.
- Lead holistic care planning using comprehensive patient assessments and evidence-based practices to deliver personalised, high-quality care.
- Coordinate complex care delivery across multidisciplinary teams, working closely with Allied Health Professionals to ensure seamless, integrated support.
- Act as a key liaison for families, providing clear, compassionate communication and emotional support throughout the care journey.
- Oversee staff scheduling and resource allocation, ensuring appropriate coverage and continuity of care at all times.
- Drive clinical quality and safety through regular audits, incident reviews, and continuous improvement initiatives aimed at improving outcomes.
- Facilitate regular stakeholder meetings, building and maintaining strong working relationships with external agencies, care providers, and regulatory bodies.
Essential Requirements
- Current NMC registration with substantial clinical experience in complex care settings, including managing patients with long-term conditions and multiple comorbidities.
- Proven leadership skills with the ability to supervise, mentor, and support the professional development of nursing and care staff.
- Strong competency in workforce planning, including staff scheduling, resource allocation, and ensuring adequate clinical coverage.
- Exceptional communication skills for collaboration with Allied Health Professionals and clear, empathetic liaison with families.
- Experience in staff competency assessment with the ability to deliver constructive feedback and develop tailored training and improvement plans.
- Key clinical skills: proficient and confident in managing PEG feeding, tracheostomy care, and Cough Assist (as applicable to the role).
