An exciting opportunity has arisen for a key member of senior staff within the Patient Safety and Assurance team. The role is diverse and touches upon a number of key health board functions including patient safety, incident management, assurance processes and regulator inspection action planning activity.
Main Duties
- Support the co‑ordination and oversight of the management of the investigation process of all serious patient safety incidents and high‑impact incidents.
- Co‑ordinate, undertake and advise on patient safety incident and other complex investigations or thematic reviews, including working with external reviewers.
- Manage the Duty of Candour process for allocated incidents.
- Provide specialist advice and support to the Triumvirate Team and others on all issues relating to risk and safety.
- Undertake patient safety incident and other complex investigations or reviews, ensuring they are managed in line with good practice and established project management methodologies.
- Utilise advanced clinical skills and extensive knowledge base to undertake specialist assessments, critically analysing complex information to make informed decisions regarding material to inform investigations.
- Deputise for the Patient Safety and Assurance Manager on occasion as required, ensuring continuation of the department's functions.
Responsibilities
- Support the Patient Safety and Assurance Manager in the oversight of responses to external inspections and peer reviews, ensuring action plans are SMART.
- Day‑to‑day management of allocated caseload, managing multiple complex investigations and programmes of work.
- Support the Quality Assurance and Safety Team in identifying learning themes and feeding back outcomes of investigations.
- Undertake, review and analyse highly complex, sensitive investigations and interpret and apply Health Board policies, procedures, regulations and relevant statutory obligations.
Person Specification
Qualifications and Knowledge
- Relevant professional qualification with current professional registration.
- Educated to Master’s level, or equivalent experience.
- Evidence of continued professional development.
- Working knowledge of relevant literature, research and legislation relating to clinical governance, patient safety and quality.
- Clear understanding of evidence‑based practice and clinical effectiveness.
- Trained in recognised investigation methodologies (e.g. 'Root Cause Analysis techniques').
- Conversant with the duties under the Health and Social Care (Quality and Engagement) (Wales) Act 2020 and the Health and Care Quality Standards.
- Conversant with Putting Things Right Regulations.
Desirable criteria – Qualifications and Knowledge
- Interest and knowledge in the broad healthcare agenda.
Experience
- Significant post‑registration experience.
- Undertaking complex investigations and being a lead Investigation Officer.
- Incident investigation and investigation report writing skills.
- Risk management / clinical governance / patient safety and quality.
- Use of risk assessment tools, methodologies and risk management.
- Managing change.
- Multi‑professional team working.
Desirable criteria – Experience
- Experience at a senior level.
