Location: North-west (Home based with regional travel)

Salary: £45k - 55k, depending on experience

Role can be offered on a full-time or part-time basis (min 3 days per week) or a Zero Hours Contract basis.

Our Specialist Rehabilitation Case Managers take on a caseload of complex Catastrophic injury cases and their focus is the timely and effective coordination of treatment and support to maximise their recovery outcomes. Our Case Managers assess and understand an injured person's specific needs and use their clinical and professional skills to ensure a maximal recovery. This extends beyond the individual's presenting physical and functional abilities, and encompasses all aspects of their social and vocational situations and life participation. We are now looking for a qualified clinician (such as a Physiotherapist, Occupational Therapist or Nurse) with demonstrable experience of dealing with spinal cord injuries. Brain or neurorehabilitation experience also very beneficial.

Key Responsibilities:

  • To proactively manage a caseload of rehabilitation clients from referral and assessment through active case management to case closure within an agreed geographical location.
  • Caseload numbers vary depending on case complexities.
  • To be responsible for assessing and developing rehabilitation plans for clients.
  • To be responsible for liaising and maintaining regular communication with the rehabilitation client, case stakeholders and clinicians, keeping all case records accurate and up to date.
  • To accurately request, agree and manage funds allocated for rehabilitation on each case, reviewing and processing treatment invoices and monitoring overall case spend.
  • To work towards agreed monthly financial and revenue targets, following HCML policies and guidelines.

Requirements Key Skills and Experience:

Essential:

  • Minimum of five years of clinical experience in the field of neurological and complex orthopaedic rehabilitation, including spinal cord injury.
  • An understanding of statutory services provisions in relation to life-changing injuries.
  • An awareness and understanding of NICE guidelines in reference to neurological injury to allow for thorough assessment and rehabilitation coordination and planning including MDT collaboration.
  • Excellent written skills compilation of extensive reports, attention to detail, professional email communication.
  • Willingness to travel to hospital, residential and clinic locations under own means or via public transport (travel expensed).
  • Qualification in Nursing, Physiotherapy or Occupational Therapy.
  • Active HCPC registration, professional board membership.

Desirable:

  • Experience in the field of rehabilitation case management.
  • Experience in or understanding of the personal injury rehabilitation market.
  • Experience of commissioning rehabilitation services either statutory or private.
  • Experience of developing a rehabilitation plan and making referrals.
  • Specialist post graduate training in spinal injuries.
