Role Overview

As a Clinical Rotational Pharmacist, you work across multidisciplinary teams, taking ownership of medicines optimisation within your designated clinical area.

Responsibilities

  • Deliver specialist clinical pharmacy services to ensure safe, effective and economical use of medicines.
  • Support Lead Pharmacists and the Clinical Pharmacy Manager in providing a high‑quality, patient‑focused service aligned to national standards.
  • Conduct medication reviews, reconciliation on admission, and pharmaceutical care planning for discharge.
  • Provide expert clinical advice during ward rounds and across healthcare teams.
  • Advise on complex prescribing and ensure compliance with best practice and legislation.
  • Contribute to formulary decisions and support the Drugs & Therapeutics Committee.
  • Lead medicines optimisation initiatives and clinical audits.
  • Contribute to development and implementation of local guidelines and policies.
  • Develop others by delivering training and sharing knowledge across medical, nursing, and pharmacy teams.

You'll be at the heart of patient care, directly influencing treatment decisions, improving patient safety and reducing medication‑related risks across the organisation.

Through your specialist clinical expertise, you help ensure patients receive the most effective pharmaceutical care with the best possible outcomes and minimal adverse effects.
