Heald Green Health Centre is looking for an experienced and dynamic Advanced Nurse Practitioner (ANP) to join our friendly, supportive, and forward-thinking primary care team. This is an exciting opportunity to work autonomously within a high-quality general practice environment, providing expert clinical assessment, diagnosis, treatment, prescribing, and long-term condition management. Main duties of the job

  • Provide high-quality autonomous clinical assessment, diagnosis, treatment, and management of undifferentiated, acute, urgent, and chronic presentations.
  • Undertake acute illness and disease management, including prescribing and medication reviews within scope of practice.
  • Offer same-day care and support urgent on-the-day demand as part of the duty/on-call team.
  • Provide first-contact care for minor illness, injury, and undifferentiated conditions.
  • Request, interpret, and action investigations as appropriate.
  • Support proactive management of long-term conditions, chronic disease reviews, and preventative healthcare.
  • Deliver face-to-face, telephone, and electronic consultations depending on practice needs.
  • Liaise with other health and social care professionals and make appropriate referrals.
  • Participate in public health and screening programmes including immunisations and vaccinations.
  • Contribute to clinical governance, service development, quality improvement, and care pathway development.
  • Provide leadership, mentoring, and specialist clinical advice to colleagues across the multidisciplinary team.

Job responsibilities

  • Act autonomously within professional boundaries to assess, examine, diagnose, treat, and evaluate patients.
  • Use advanced clinical reasoning and critical thinking to support safe decision-making.
  • Provide expert management of acute and chronic conditions, using both drug and non-drug treatment options.
  • Undertake comprehensive clinical assessments including physical and psychological evaluation.
  • Proactively identify, diagnose, and manage patients at risk of long-term health conditions.
  • Prescribe and review medication safely and effectively in line with national/local guidelines.
  • Manage urgent or emergency situations promptly and appropriately.
  • Prioritise patient problems, triage effectively, and intervene as needed.
  • Support patients with health promotion, preventative advice, lifestyle change, and self-management.
  • Work collaboratively within the multidisciplinary team, contributing to policy, protocols, audits, and service improvement.
  • Maintain accurate, contemporaneous clinical records using electronic systems and SNOMED coding.
  • Provide clinical advice, support colleagues, and mentor junior staff.
  • Contribute to safeguarding, mental health recognition, and appropriate referral for vulnerable patients.
  • Communicate sensitively and effectively with patients, carers, and staff, adapting communication styles as necessary.
  • Uphold equality, safeguarding, confidentiality, and professional standards at all times.

Person Specification

  • Registered Nurse (NMC).
  • MSc or Postgraduate Diploma in Advanced Clinical Practice (or equivalent experience).
  • Evidence of ongoing CPD.
  • Advanced clinical assessment/decision-making training.
  • Expertise or interest in specific long-term conditions (e.g., asthma, COPD, frailty, diabetes).

Experience

  • Proven ability to assess and manage acutely unwell and chronically ill patients in primary care.
  • Experience working in general practice and within multidisciplinary teams.
  • Experience in triage, prescribing, and autonomous clinical decision-making.
  • Experience with chronic disease management and medication reviews.
  • Excellent clinical, communication, and decision-making skills.
  • Minimum 2 years post-qualification as an ANP or equivalent role.
  • Supervising students or junior clinicians.
  • Experience with EMIS Web.
  • Learning disability reviews, dementia reviews, COIL fitting/removal.
  • Background in primary care nursing.
