We are seeking a dynamic, patient-focused Advanced Practitioner (AP) with a specialist interest in rehabilitation to join the MDT on Oxford and Cambridge Ward at Herts & Essex Hospital.

Oxford and Cambridge Ward is a specialist rehabilitation unit providing assessment, treatment and rehabilitation for patients with complex needs, including frailty, deconditioning, falls and long-term conditions. The ward supports patients to maximise independence and achieve rehabilitation goals, enabling safe and timely discharge.

Working closely with the Ward Leadership Team and multidisciplinary colleagues, you will provide expert clinical leadership, deliver high-quality patient-centred care, support clinical decision-making, and contribute to service development and quality improvement. The role includes collaborating with medical teams to assess, review and manage patients, developing clear management plans and utilising advanced clinical skills to support assessment, diagnosis and treatment.

This is an exciting opportunity to support the ongoing development of rehabilitation services within HCT. The post holder may occasionally provide support across other inpatient rehabilitation units, including QVM Hospital and Danesbury Neurological Centre, ensuring consistent, high-quality care across the service.

Main duties of the job

  • Carry out weekly ward round alongside the ward GP and Speciality doctor.
  • Prescribe and review medication as per required/appropriate for patients on the unit.
  • Interpret/request blood tests where appropriate for patients on the unit.
  • Interpret/request other investigations.
  • Develop medical treatment plans for patients on the unit.
  • Provide supervision and support for the nursing team on the unit.
  • Coach staff both internal to HCT and external seeking advice on patients’ suitability for admission to the unit and liaise with the acute trusts.
  • Support in the development/training of ACPs across HCT.
  • Work alongside the project development team in HCT.
  • Communicate with other HCT Inpatient units to ensure appropriate medical/ACP cover across all three sites.

Detailed job description and main responsibilities

The post holder will apply advanced clinical expertise, levels of judgement, discretion and decision making. Will act as an autonomous practitioner working independently and will exercise expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with neurological needs, complex presentations and multi pathologies.

The role will provide an expert resource, providing a visible clinical profile, having both advisory and clinical input into patient care. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

Works collaboratively with the multi-disciplinary team including medical, nursing and allied health staff to manage medical conditions and to plan and implement treatment for illnesses.

As an Advanced Clinical Practitioner, this role will provide education to other staff and students within the clinical area.

Person specification

Qualifications and training

  • Currently registered practitioner with relevant Professional Governing Body
  • First Degree or equivalent experience
  • Master’s Degree in Advanced Practice or equivalent experience
  • Post-Graduate – Extended Independent and Supplementary Non-Medical Prescribing.
  • Appropriate Post-Registration specialist education / qualifications or equivalent, examples could include (not exclusive) in elderly care, long term conditions, neurological care, rehabilitation, medical care, advanced assessment, etc

Experience and Knowledge

  • Needs significant post registration experience commensurate with being an Advanced Practitioner and demonstrate competence and experience in clinical management
  • Minimum of 5 years post registration experience.
  • Minimum of 2 years at band 6 or higher in a relevant specialty
  • Evidence of leading organisational and management change
  • Working with other stakeholder organisations
  • Experience of working within a community setting

  • Effective communication skills, including the ability to communicate complex and highly sensitive information
  • Excellent problem-solving skills
  • Ability to synthesise information, consider & evaluate risks & options when making difficult decisions
  • Able to maintain judgement under pressure

  • Ability to network with other AP's and clinical colleagues, both within and external to HCT
