Are you ready for the next chapter in your ACP journey? Wiltshire Health and Care have an exciting full-time opportunity for a fully qualified or trainee ACP to join the Hospital at Home team at Wiltshire Health and Care. You will be based in north Wiltshire alongside a clinical team comprising Consultant Practitioners/Associate Specialists/GP trainees and other ACPs supporting the team with the medical aspects of patient care. Applications are invited from candidates who are a Registered Nurse or Health Care Professional, holding a MSc in Advanced Practice (or working towards) and have relevant post-registration experience. Experience at Band 6 or above with knowledge of effective change management skills and a proven track record in the delivery of high-quality clinical care and Non-Medical Prescribing are essential.

Main Responsibility

  • To deliver a clinically autonomous Advanced Clinical Practitioner service to the Community Teams in line with national and organisational standards – Urgent Crisis Response, admission avoidance, focus on frailty.
  • To assess and manage patients on a virtual ward (Hospital @ Home) with a variety of complex health issues to avoid unnecessary acute care admissions.
  • The post holder will be able to combine advanced clinical expertise and assessment to establish a diagnosis, including referral for investigation, intervention and treatment with minimal avoidable delay and lead the co-ordination of care working with partner agencies and key stakeholders.
  • Will be an expert resource, providing a visible clinical patient profile (90%) and have management and advisory responsibilities.
  • The post holder will put the patient at the centre of all decisions taking into consideration any patient decisions regarding ongoing and future care management.
  • Negotiate and agree with the patient, carers and other care professionals, individual roles and responsibilities with actions to be taken and outcomes to be achieved, referring on to other services or professionals as appropriate.
  • To provide professional/clinical leadership to the Nursing or AHP staff in the Community Team as and where appropriate/agreed.
  • To provide high standards of care to patients and support the Community Team Business Manager in improving quality of care.
  • Where appropriate avoid unnecessary transfer of the patient to acute secondary care.
  • To be a visible leader in the Community team working in conjunction with the nursing and therapy teams.

The Ideal Candidate

We are looking for an enthusiastic and dynamic individual with clinical expertise and a passion for care. The successful candidate will be a team player who is flexible and adaptable to the needs of the service, who enjoys working in a multidisciplinary team delivering high-quality care.

Essential

  • Registered with the NMC or HCPC.
  • First level nursing, Physiotherapy, Occupational therapy, or Paramedic degree.
  • Recognised teaching or mentorship qualification equivalent to level 3.
  • History Taking and Physical Assessment module at Masters level.
  • Diagnostics module at Masters level.
  • MSc in Advanced Clinical Practice or willingness to work towards.
  • Previous experience in gerontology or a relevant medical speciality at band 6 or above.
  • Evidence in challenging clinical practice and leading in the development of new pathways of care and delivering best practice.

  • A clean driving licence, access to a vehicle for daily use.
