Social Worker – Home First Neighbourhood South Team in Didcot, Oxfordshire
This is a full time temporary position on ongoing basis.
Focussing on supporting hospital discharges from mental health hospitals.
Improving mental health outcomes, bringing a unique social and rights based perspective to a dynamic multi-disciplinary team. Ensuring that people receive the right support when discharged from hospital to enable them to thrive in their communities.
Responsibilities:
- Completing Care Act Assessments, Mental Capacity Assessments and S117 Assessments.
- Engaging in the Best Interest Process.
- Drafting Social Circumstances Reports for Mental Health Tribunals.
- Collaborating with health professionals, individuals, and their families.
- Attending Multi-Disciplinary Team meetings at the Fullbrook Centre.
- Developing support plans tailored to individual needs.
Requirements:
- Knowledge and understand of key legislation in Adult Social Care including The Care Act, The Mental Capacity Act and The Mental Health Act
- To undertake holistic, strength based assessments.
- To create personalised care and support plans
- To identify and respond appropriately to Safeguarding concerns
- To work closely with other agencies such as The NHS and care providers
- To advocate for people you are working with
- Strong organisational and time management skills
