Salary: £34.35 per hour

Job Type: Full time, Temporary

Hours: 9am to 5pm

About the Role: As an Adults Social Worker, you will carry out independent assessment and support planning with the use of current legislation, theoretical models and best practice approaches.

Responsibilities:

  • Independent assessment and Support Planning
  • Work within the Transfer of Care hub for multi-disciplinary discussions and social work interventions
  • Support and facilitate during hospital admission and hospital discharge
  • Work in the community
  • Undertake strength-based assessment and advocating support for people after discharge

Who We Are Looking For:

Education: Degree in Social Work (or equivalent)

Licenses: Social Work England registered

Experience: Minimum 3 years post qualified experience in direct employment with an English Local Authority
