We have an exciting opportunity for an Advanced Clinical Practitioner to join our West Wiltshire Locality Hospital at Home service. The service is currently undergoing an exciting period of transformation, making this an excellent time to become part of a dynamic and innovative team. You will be based in our Trowbridge Hospital location but work with the integrated community health & care services and people’s homes. Service hours run from 8am-8pm, 7 days a week.

As an ACP, you will play a pivotal role in providing expert clinical assessment, diagnosis, treatment, and management of patients within your area of specialty. You will lead by example, supporting colleagues, championing best practice, and helping drive forward innovation in patient pathways.

Main Responsibilities

  • Deliver a confident, compassionate ACP service that supports crisis response, admission avoidance, and care for people living with frailty.
  • Provide thoughtful assessment and management for patients on the virtual ward, helping them remain safely at home.
  • Use advanced clinical skills to reach timely diagnoses and arrange investigations and treatments, working closely with partner agencies for seamless care.
  • Act as a visible and approachable clinical expert, offering direct patient care alongside guidance and support to colleagues.
  • Place patients at the centre of all decisions, respecting their preferences and supporting shared decision‑making.
  • Work collaboratively with patients, carers, and professionals to plan care and make appropriate referrals.
  • Offer supportive leadership to Nursing and AHP colleagues within the Community Team.
  • Provide high‑quality care and contribute to ongoing quality improvements.
  • Help prevent unnecessary hospital transfers by offering safe and effective alternatives.
  • Be a positive, accessible leader within the Community Team, promoting joined‑up working across nursing and therapy teams.
  • Work closely with primary care to support safe planning of care at home and reduce readmissions.
  • Communicate effectively with inpatient teams to support safe discharge and clear follow‑up.
  • Maintain professional accountability and uphold all relevant codes, standards, and organisational policies.
  • Keep clear, accurate, and up‑to‑date clinical records.
  • Support organisational goals, including participating in screening and assessments where needed.
  • Provide guidance to colleagues involved in health promotion and chronic disease management.
  • Stay up to date with research, sharing learning and helping embed evidence‑based practice.
  • Identify risks or opportunities for service development and share recommendations with the line manager.
  • Take responsibility for ongoing professional development to maintain and enhance clinical competence.
  • Support the effective use of resources and contribute to budget discussions with the Community Team Business Manager.
  • Build strong, positive relationships with internal colleagues and external partners across health and social care.
  • Contribute to shaping and evolving the ACP role as service needs change.
  • Take part in audits, data collection, and benchmarking to inform future service improvement.

The Ideal Candidate

Essential

  • First level nursing, Physiotherapy, Occupational therapy, or Paramedic degree
  • Recognised teaching or mentorship qualification equivalent to level 3
  • History Taking and Physical Assessment module at Masters level
  • Diagnostics module at Masters level
  • MSc in Advanced Clinical Practice
  • MSc in Advanced Clinical Practice or willingness to work towards
  • Previous experience in gerontology or a relevant medical specialty at band 6 or above.
  • Evidence in challenging clinical practice and leading in the development of new pathways of care and delivering best practice
  • Evidence of successfully resolving complex problems
  • Experience in carrying out clinical audit and risk assessment
  • Proven skills in leading, managing and developing teams
  • Evidence of successful resource management.
  • Highly specialist clinical and technical skills in managing a range of clinical conditions and end of life care
  • Strong analytical and judgement skills, with the ability to interpret complex situations and make appropriate decisions.
  • Excellent interpersonal skills and ability to build and maintain relationships with other health and social care professionals, patients, carers and relatives.
  • Excellent written and verbal communication skills
  • Ability to develop and deliver training packages to a wide range of staff
  • Ability to work autonomously without supervision
  • Ability to meet deadlines and work under pressure
  • Ability to adapt to the demands of a constantly changing environment
  • Good IT skills
  • IT literate
  • Willingness to work in other areas of the service as and when required to do so.
  • Current full driving license and use of a car during working hours.

Desirable

  • Independent Non-Medical Prescriber Qualification
  • Completion of required modules for Band 7 Clinical leadership
  • Experience as an Advanced Clinical Practitioner/ Clinical Lead.
  • Strong IT Skills – including excel and power-point
  • Knowledge of national NHS drivers and initiatives
  • Advanced clinical practice skills such as interpretation of ECG; X-rays.
  • Presentation skills
  • Ability to plan and manage resources within allocated budget effectively
  • Knowledge of national and local NHS healthcare agenda, standards, research and its implications. Active in national clinical networks, involving care of older people
