HCRG Care Group is seeking an Enabler to join the Intermediate Care and Reablement Service in Sittingbourne. You will work with a multidisciplinary team to support people to regain independence and achieve individual reablement goals in the community.
You will travel between visits, assist with personal care, mobility and daily activities, and contribute to therapy and rehabilitation plans while maintaining high standards of care and documentation.
