We're looking for an experienced and enthusiastic Band 6 Physiotherapist to join our exciting Rapid Response and Intermediate Care Team to assess and treat patients within the local community. The Physiotherapist would be working closely with Physiotherapy colleagues as well as working collaboratively with our Occupational Therapists, Nurses and Support Worker colleagues. The service operates 7 days a week 8am till 8pm, and so includes weekend and bank holiday working. The team is headed by the Clinical Service Manager for Swale Rapid Response and Intermediate Care, and has 3x Clinical leads in post for nursing, physiotherapy, and occupational therapy. They provide line management and clinical support to the team.
Main duties of the job
- Work within Rapid Response and Intermediate Care across the Swale locality
- Complete complex holistic initial assessments with patients with a wide range of physical conditions
- Triage patient referrals from a variety of sources
- Liaise with family members, carers, and wider support services
- Devise and review care plans
- Delegate rehabilitation programmes to support worker colleagues
- Complete follow up visits with patients
- Attend and participate in clinical meetings, team meetings, service development workstreams
- Provide training and clinical support to colleagues
- Offer and support student placements from local universities
- Supervise and appraise staff within your line management
