HCRG Care Group in Surrey is seeking an experienced Transition Lead Nurse (Band 7) to lead a high-profile programme focused on improving the experience and outcomes of young people transitioning from children’s to adult health and care services.
You will develop, implement and embed integrated transition pathways, SOPs and guidelines, working with health, social care, education and voluntary sector partners to drive service improvement.
This is a 12-month part-time fixed-term role.
