The integrated Community Neurology and Stroke Service are looking for an enthusiastic RSW to support the team in delivering rehabilitation to patients with neurological conditions with their own homes. Previous experience would be beneficial, but not essential.

Rehabilitation Support Worker (RSW) WTE 0.56 21 hours/Week

Location: Trowbridge or Melksham

The integrated Community Neurology and Stroke Service are looking for an enthusiastic RSW to support the team in delivering rehabilitation to patients with neurological conditions with their own homes. Previous experience would be beneficial, but not essential.

You will be working with a supportive multidisciplinary team comprising Occupational Therapists and Physiotherapists and within the wider team working with a range of providers and disciplines.

The team see a variety of patient conditions providing varying stages of rehabilitation and maintenance input, including end of life support.

This role will be based in the community in West Wiltshire although a degree of flexibility will be required depending on service demands.

This is an exciting opportunity to work with our dedicated and experienced therapists. As well as receiving regular supervision, there is a regular programme of in service learning, peer support and opportunities for service development.

A full valid driving licence and access to a car to use regularly for business purposes is essential.

Main Responsibility

Your days will be rewarding and varied and include:

  • To work within a multi- disciplinary team.
  • To implement treatment by following care plan identifying rehabilitation goals.
  • To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
  • To undertake the daily rehabilitation and support of delegated patients.
  • Deliver treatment within a care home or community setting as specified in a written care plan, under the guidance of a registered Occupational and/or Physiotherapist.
  • To practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory body’s guidelines.
  • Wherever possible, to promote independent function throughout the patient’s stay in a care home or in the persons own home, ensuring that the patient receives the appropriate treatment in the most appropriate setting.

Patient Care

  • Promote the independent function in all daily tasks of patients designated by the registered practitioner.
  • Take responsibility for following identified OT and PT rehabilitation goals; to monitor progress and provide regular and comprehensive feedback.
  • To support patients to practice mobility and exercise techniques as rehabilitative goals.
  • To have a good knowledge of infection control, to reduce risk of health care acquired infections.

The Ideal Candidate

The ideal candidate will have the following:

  • Good general education.
  • NVQ Level 3 in health care (or willingness to undertake).
  • Food Hygiene certificate (or willingness to undertake).
  • Experience of working with older adults
  • Understanding of confidentiality issues.
  • Ability to work as a team member.
  • Delivering patient focused care.
  • Ability to recognise own limitations.
  • Typing/data inputting skills.
  • Current full driving license and use of a car.
  • Willing to work in other areas as and when required to do so.
  • Self-motivated.
  • Good time management
